“When I just started working long time ago, I heard people said that hospital was the most complicated building to build. I remembered it well and found the truth when working with Bangkok Hospital. To build a hospital was involves with ‘Life support’ both technology and personnel; ‘Safety’ for service users and staffs in the hospital as well as ‘Security’ to prevent the risks. These are main factors that need to be considered.
For affiliated hospitals of Bangkok Hospital, project developments are organized into categories based on the potential of areas and cities. For the business, we divide levels of medical care into Secondary Care, Tertiary Care and Super Tertiary Care. We set up a Secondary Care for a town and Tertiary Care for a city. We have only Super Tertiary Care in a capital city as Bangkok which is Bangkok Hospital (Soi Soonvijai, Bangkok). The hospital is not just a building, but also includes equipment and medical specialists in various fields therefore it becomes a complete medical center. Moreover, to categorize the levels of medical care can help us manage construction costs efficiently.
For Bangkok Hospital and affiliated hospitals, a design principle is based on the simplicity and uncomplicated. A simple design can bring a high performance to the buildings, for example we created an existing space with maximum functions as each square meter means the budget. However, the main facilities are essential to invest for optimizing efficiency and services such as ER, OR, MRI and X-ray machines, etc. For the design, we select building materials by based on patients’ expectation, also the healthcare standard by the Ministry of Health as well as the standard of manufacturers. For our OR, there is not differing from other hospitals in a group of affiliated because safety is our first priority.”

“สมัยทำงานใหม่ๆ จะได้ยินคนในวงการเดียวกันพูดว่า ตึกที่สร้างยากที่สุดก็คือตึกของโรงพยาบาล ส่วนตัวก็จดจำคำกล่าวนี้ได้ดี และได้เห็นความจริงเมื่อตอนที่เราเริ่มเข้ามาทำงานกับโรงพยาบาลกรุงเทพเนื่องจากการสร้างโรงพยาบาลนั้นเกี่ยวข้องกับ Life Support ทั้งเทคโนโลยีและบุคลากรที่เกี่ยวข้องกับชีวิต Safety เรื่องความปลอดภัยของผู้คนที่มาใช้บริการและบุคลากรที่ทำงานอยู่ภายในไปจนถึง Security ระบบรักษาความปลอดภัยจากมิจฉาชีพต่างๆ ซึ่งเหล่านี้ถือเป็นปัจจัยหลักที่จำเป็นต้องระลึกถึง
สำหรับโรงพยาบาลในเครือโรงพยาบาลกรุงเทพ การพัฒนาโครงการของโรงพยาบาลถูกจัดออกเป็นหมวดหมู่ตามศักยภาพของพื้นที่ ในการสร้างโรงพยาบาลในพื้นที่ต่างๆ เราจะมองศักยภาพของเมืองซึ่งที่จริงแล้วในธุรกิจโรงพยาบาล เราจะแบ่งขั้นของโรงพยาบาลออกเป็น ขั้นทุติยภูมิ (Secondary Care) คุณอาทิตย์ ลิ้มไพศาล ผู้ช่วยกรรมการผู้อำนวยการใหญ่อาวุโสด้านพัฒนาอสังหาริมทรัพย์ บริษัท กรุงเทพดุสิตเวชการ จำกัด (มหาชน) โรงพยาบาลกรุงเทพ และโรงพยาบาลในเครือขั้นตติยภูมิ (Tertiary Care) และขั้นตติยภูมิระดับสูง (Super Tertiary Care) ดังนั้นหากเป็นหัวเมืองเล็กๆเราก็จะวางให้เป็น Secondary Care ส่วนหัวเมืองใหญ่จะถูกวางเป็น Tertiary Care ส่วน Super Tertiary Care จะมีเพียงที่เดียว คือ ที่โรงพยาบาลกรุงเทพ (ซอยศูนย์วิจัย กรุงเทพฯ) เพราะโรงพยาบาลไม่ได้มีเพียงแค่ตึกเท่านั้น แต่ยังมีเครื่องไม้เครื่องมือและบุคลากรทางการแพทย์ที่เชี่ยวชาญในสาขาต่างๆ รวมอยู่ด้วย จึงจะกลายเป็นพื้นที่บริการทางการแพทย์ที่สมบูรณ์ ดังนั้นการแบ่งขั้นที่ชัดเจน ย่อมมีผลในแง่การลงทุนก่อสร้างซึ่งจะทำให้เราสามารถบริหารและจัดการได้อย่างมีประสิทธิภาพ
สำหรับโรงพยาบาลกรุงเทพและโรงพยาบาลในเครือนั้น เมื่อจะออกแบบอาคารเรามักจะคำนึงถึงความเรียบง่ายและไม่ซับซ้อนเป็นหลัก การออกแบบที่เรียบง่ายจะทำให้ประสิทธิภาพของอาคารมีสูงอย่างเรื่องการใช้พื้นที่ที่มีอยู่ให้คุ้มค่าและได้ประโยชน์สูงสุด ด้วยพื้นที่ทุกๆ ตารางเมตร ล้วนหมายถึงการลงทุนทั้งสิ้น แต่ทั้งนี้การลงทุนสำหรับโครงสร้างพื้นฐานจำเป็นต้องมีให้ครบไม่ว่าจะเป็นห้อง ER ห้อง OR ไปจนถึง MRI และเอกซเรย์ เป็นต้น
ส่วนในการออกแบบและเลือกใช้วัสดุ ปัจจัยหนึ่งก็ขึ้นอยู่กับความคาดหวังของคนไข้ที่เข้ามาใช้บริการในโรงพยาบาลด้วย แต่ก็ต้องคำนึงถึงสิ่งจำเป็นพื้นฐานที่ได้มาตรฐานตามที่กระทรวงให้การยอมรับ รวมถึงมาตรฐานของบริษัทผู้ผลิตเป็นต้นว่ากระเบื้องประเภทไหนสามารถใช้ในพื้นที่ใดได้บ้าง พื้นที่ในส่วน OR ของเรานั้นอาจจะไม่แตกต่างกันมากนักสำหรับโรงพยาบาลในแต่ละกลุ่ม ทั้งนี้เนื่องจากเราตระหนักว่าเรื่องของความปลอดภัยเป็นสิ่งจำเป็นและสำคัญอย่างมาก”

An experience in design, corporate office and hospital, has brought Khun Pairoj Paisarn to be part of Planning and Development Project team at Bamrungrad International Hospital. That makes he found difficulties, simplicities and challenges in hospital project development.
“Our department is responsible for project management. Our team came from different knowledge and skills as a kind of property developments. Hospital is complicated building that bring problems to be faced. So our duty is to find an easier solution by interpreting executive’s policies into actions, in terms of schedule management and working performance.
Renovated and decorated Projects are our duties also. Many times we have adjusted layout and functions in the building for more appropriate space and changed engineering systems with new technology and energy-saving ones. If a new building, we have a strongly policy in order to meet LEED certification standards. However, the LEED strategies and practices have been gradually implemented. Now we are focusing on energy saving of M&E systems by using more automation system or energy saving systems that we have been researching for long time.
Construction management is an important to consider, especially during construction period, particularly with high standard hospitals. I found that most people come to Bamrungrad International Hospital with highly expectation to our premium services. Therefore, during the construction we provide the best solution to take care and facilitate collaboration participants in the hospital as well. Moreover, safety is a first priority for hospital project development. The difficulty of our tasks is to manage complicated projects by finding the right solution which has less impact to patients, service users, and hospital staffs and also allows everybody continues working in each section simultaneously.
For building materials, it depends on the design ‘mood & tone’ or concept of the hospital and executives’ guideline which can be adjustable according to their requirements. Nevertheless, the appropriated material is an important factor to consider as well. For the design, we ask designers to propose us the ideas because they have an opportunity to meet suppliers frequently then they will get latest innovation and technology. Nowadays production technology can produce higher quality materials; in terms of durability, ability to anti-bacteria which helps us to design with more creative and safer.”

“ขอบเขตในการทำงานของแผนกเราจะรับผิดชอบด้านการบริหารจัดการโครงการ โดยทีมงานจะมีพื้นฐานทางการศึกษาและความถนัดที่แตกต่างกันซึ่งที่จริงแผนกของเราก็เหมือนกับการพัฒนาอสังหาริมทรัพย์รูปแบบหนึ่ง ซึ่งอาคารประเภทโรงพยาบาลนั้นมีความซับซ้อนและอาจมีปัญหาให้แก้ไขอยู่เสมอ หน้าที่ของเราจึงเป็นการทำอย่างไรให้ทุกอย่างเป็นเรื่องที่ง่ายขึ้น โดยการนำคอนเซ็ปต์ของผู้บริหารมาดำเนินตามนโยบาย ทั้งในเรื่องการจัดการด้านเวลาและคุณภาพของผลงานมาประกอบกัน
ที่ผ่านมางานด้านตกแต่งและปรับปรุงอาคารก็เป็นงานหลักอย่างหนึ่งของเรา หลายครั้งเป็นการปรับตำแหน่งของพื้นที่การใช้งานให้มีความเหมาะสมมากขึ้น รวมถึงปรับเปลี่ยนงานระบบวิศวกรรมต่างๆ ให้ทันสมัยและประหยัดพลังงาน และหากเป็นอาคารสร้างใหม่ก็จะมีนโยบายที่ชัดเจนในการสร้างอาคารเพื่อให้ได้ตามมาตรฐานการรับรอง LEED ซึ่งในด้านนี้ทางโรงพยาบาลเราก็ค่อยๆ ดำเนินการแบบค่อยเป็นค่อยไป สิ่งที่เราคำนึงถึงในตอนนี้จะเน้นที่การประหยัดพลังงานของงานระบบวิศวกรรมเสียเป็นส่วนใหญ่ เช่นการใช้ระบบที่เป็นออร์โตเมติกมากขึ้น หรือระบบประหยัดไฟฟ้า ซึ่งที่ผ่านมาเราได้ทำการศึกษาค้นคว้ามาโดยตลอด
สำหรับการทำงานของทีมวางแผนและพัฒนาโครงการแล้ว การดูแลและจัดการระหว่างการก่อสร้างเป็นสิ่งสำคัญที่ต้องระลึกถึงอย่างมากเพื่อให้เทียบเคียงความเป็นโรงพยาบาลมาตรฐานสูง ผมมองว่าคนที่เข้ามารักษาที่โรงพยาบาลบำรุงราษฎร์ล้วนคาดหวังเรื่องบริการที่เป็นเลิศ ดังนั้นการดูแล และอำนวยความสะดวกในระหว่างก่อสร้างก็ควรตอบโจทย์และอยู่ในระดับเกรด A เช่นกัน ในการพัฒนาโครงการที่เป็น สถานพยาบาล Safety เป็นสิ่งที่ต้องคำนึงถึงเป็นอันดับแรก โดยเฉพาะโครงการที่มีคนร่วมใช้งานอยู่ตลอดเวลา ความยากของงานจะอยู่ที่การบริหารจัดการโครงการซึ่งมีความซับซ้อนว่าต้องทำอย่างไรที่จะทำให้คนไข้ ผู้มาใช้บริการ รวมถึงบุคลากรภายในโรงพยาบาลได้รับผลกระทบน้อยที่สุด และสามารถทำงานในแต่ละส่วนได้ในช่วงเวลาเดียวกัน
สำหรับการเลือกวัสดุในการก่อสร้างนั้น หากพูดถึงความสวยงามก็คงจะไม่แตกต่างกันมาก ทั้งนี้ขึ้นอยู่กับรสนิยม Mood & Tone รวมถึงแนวทางของโรงพยาบาลหรือแนวทางของผู้บริหารซึ่งสามารถปรับเปลี่ยนได้ตามความต้องการแต่ทั้งนี้ความเหมาะสมของวัสดุก็เป็นสิ่งสำคัญที่ต้องคำนึงถึงเช่นกัน ซึ่งในการออกแบบทางเราได้ให้ทางผู้ออกแบบเป็นผู้ให้คำแนะนำเนื่องจากมีโอกาสได้พบกับซัพพลายเออร์บ่อยครั้งกว่า ทำให้ได้พบเทคโนโลยีที่ใหม่และทันสมัยกว่า ปัจจุบันเทคโนโลยีในการผลิตทำให้วัสดุมีคุณภาพสูงขึ้นทั้งในแง่ของความคงทน รวมถึงความสามารถในการป้องกันแบคทีเรีย ซึ่งสามารถช่วยให้การออกแบบสามารถทำได้หลากหลายและปลอดภัยขึ้น”

“Mostly, we are focusing on hospital design which can be divided into 2 target groups; Fee-for-Service and Capitation. The Fee for Service is hospitals for patients who pay in cash or pay by third party or insurance company. The design will focus on facilities and elegant atmosphere for serving to these customers. Capitation is hospitals for patients who have Social Security Healthcare by government. The design will has different decoration to meet the requirements of Group B and lower target customers.
Currently, our hospital design concept is emphasized on approaching process by creating a characteristic of warm-welcome and memorable. New hospitals are always designed by focusing on buildings’ appearance. But we are focusing on specialized clinics for grouping all patients and also considering on open space or creating volumes of spaces to provide a comfortable atmosphere. Besides, to create an internal atmosphere is necessary as well as to select the suitable materials either. Since the decorative material can represent a taste, theme of hospitals and including clinic center.
To build a hospital is not only to create a space for functions, but also to create a nice atmosphere for patients and service users. We have tried to design hospitals where patients can perceive more spacious atmosphere, the warm feeling of OPD and IPD and change experience of old traditional atmosphere in hospital. Moreover, we have tried to design spaces based on a group of users. For specialized clinic centers, we have created each clinic in a different atmosphere. For Children’s clinic, we have created an atmosphere of mother and child which the decorative materials must be suitable for users as well.
Particularly, we focus on material selection. For Example the latest affiliated hospital that we constructed is Sukhumvit Hospital where we considered on the building’s appearance as well as we pay attention to material selection inside the building. We use hygienic materials which are good for patients, service users, including easy maintenance. As we use vinyl tiles for purpose of easy maintenance. Due to new technologies and innovations, more products are supportive on hygienic and anti-bacteria. We can use these products for OPD, wards, including Corridors. For a design of the products, nowadays there are various designs and collections such as vinyl flooring with wood pattern that we can be used in many areas. However, the important factors to consider for material selection are aesthetic, easy maintenance and reasonable price.”

“ในส่วนของบริษัท ผลงานการออกแบบหลักของเราในตอนนี้เน้นไปทางโรงพยาบาลเป็นส่วนใหญ่โดยจะแบ่งเป็น 2 กลุ่ม ได้แก่ กลุ่ม Free for Service นั่นคือโรงพยาบาลสำหรับกลุ่มคนไข้ที่ชำระค่าบริการเป็นเงินสดหรือจ่ายโดย Third Party หรือบริษัทประกัน ซึ่งการออกแบบจะต้องคำนึงถึงการมี Facilities และบรรยากาศที่สัมพันธ์กับผู้ใช้บริการกลุ่มนี้ ส่วนอีกกลุ่มคือ Capitation หรือโรงพยาบาลที่มีการเบิกจ่ายโดยรัฐบาล อาทิ คนไข้ประกันสังคม ซึ่งก็จะมีการออกแบบตกแต่งในอีกรูปแบบหนึ่งตามความคาดหวังของกลุ่มลูกค้าที่อยู่ในกลุ่ม B ลงมา
ปัจจุบันการออกแบบอาคารประเภทโรงพยาบาลเราพยายามสร้างกระบวนการการเข้าถึงเป็นสำคัญดังนั้นอาคารที่ก่อสร้างในยุคใหม่จึงถูกสร้างคาร์แรกเตอร์ให้รู้สึกเชื้อเชิญและน่าจดจำ โรงพยาบาลใหม่ๆ หลายแห่งมักจะออกแบบโดยเน้นที่รูปลักษณ์ของอาคาร รายละเอียดของโรงพยาบาลใหม่ที่เราสร้างก็จะเน้นเป็นคลินิกเฉพาะทางเพื่อGrouping คนไข้มากขึ้นด้วย โดยคำนึงถึงการเปิดOpen Space หรือการสร้าง Volume ของ Open Space ให้รู้สึกไม่อึดอัด ซึ่งเป็นเรื่องที่สำคัญและการสร้างบรรยากาศภายในอาคารก็มีความจำเป็นความสำคัญจึงอยู่ที่การเลือกใช้วัสดุด้วยเช่นกันเนื่องจากวัสดุจะบ่งบอกได้ถึงรสนิยม ธีมของโรงพยาบาล ซึ่งรวมถึงคลินิกเฉพาะทางด้านต่างๆ ด้วย
การสร้างอาคารของโรงพยาบาลจึงไม่ใช่เพียงแค่พื้นที่สำหรับการใช้งานเพียงอย่างเดียว แต่หมายถึงสร้างบรรยากาศที่ดีสำหรับคนไข้และผู้มาใช้บริการด้วย โรงพยาบาลที่เราออกแบบส่วนใหญ่เราพยายามที่จะทำให้คนไข้ที่เข้ามารับรู้ได้ถึงบรรยากาศโอ่โถงมากขึ้น ไม่ใช่บรรยากาศของความเป็นโรงพยาบาลเหมือนที่เคยรู้จักกันมาแต่อยากให้รู้สึกถึงความอบอุ่น ไม่ว่าจะเป็น OPD หรือ IPD นอกจากนี้เราก็พยายามแยกประเภทของคนไข้ อาทิ คนไข้ที่อยู่กลุ่มคลินิกเฉพาะทางที่ต้องมีการ Follow up เราก็จะทำบรรยากาศของคลินิกแต่ละคลินิกให้แตกต่างกันออกไปอย่างคลินิกเด็กก็จะมีการสร้างบรรยากาศของความเป็นแม่และเด็ก ซึ่งการเลือกวัสดุในการตกแต่งก็ต้องเลือกให้มีความเหมาะสมกับกลุ่มลูกค้าเช่นกัน
เราเน้นให้ความสำคัญกับการเลือกใช้วัสดุอย่างโรงพยาบาลในเครือแห่งล่าสุดที่เราสร้างคือโรงพยาบาลสุขุมวิท นอกจากจะต้องคำนึงถึงรูปลักษณ์ภายนอกของอาคารแล้ว สิ่งที่ต้องคำนึงถึงภายในอาคารคือ การเลือกใช้วัสดุที่ถูกสุขอนามัยที่ดีต่อทั้งผู้ป่วยและผู้มาใช้บริการ รวมถึงง่ายต่อการทำความสะอาด หรืออย่างเช่น การเลือกใช้กระเบื้องยางนั้น เราสามารถตัดปัญหาเรื่องความกังวลในเรื่องการทำความสะอาดออกไปได้เลยยิ่งในยุคปัจจุบันด้วยนวัตกรรมที่ก้าวไกล ทำให้มีผลิตภัณฑ์ที่สนับสนุนเรื่องการปลอดเชื้อและยับยั้งแบคทีเรียมากขึ้น เราจึงนำมาใช้ทั้งในส่วนของ OPD รวมถึงในวอร์ด ห้องพัก ไปจนถึงระเบียงทางเดิน ส่วนเรื่องความสวยงามของผลิตภัณฑ์นั้น ในปัจจุบันมีหลากหลายดีไซน์ให้เลือกได้ อย่างเช่นพื้นลายไม้ที่เราสามารถนำไปใช้ได้ในหลายแผนกหลายพื้นที่ แต่ทั้งนี้ปัจจัยสำคัญที่ต้องคำนึงถึงในการเลือกวัสดุก็ยังคงเป็นการเลือกวัสดุที่ดูดีดูแลรักษาง่าย และมีราคาเหมาะสม”
